Providing Compassionate Care through the We Honor Veterans Program

At VitalCaring, we recognize that veterans’ unique identities and experiences shape their end-of-life care needs. Through dedicated efforts such as veteran-centered education for our staff and volunteers, we strive to continuously deliver compassionate, personalized care, honoring those who bravely served in their time and whom we are privileged to serve now.
VitalCaring Hospice is honored to join We Honor Veterans, a program created by the National Hospice and Palliative Care Organization with the Department of Veterans Affairs. This program is dedicated to understanding and meeting the unique end-of-life care needs of our nation’s veterans.
Through our partnership with We Honor Veterans, VitalCaring Hospice has achieved level three recognition. As part of this collaboration, we have implemented several initiatives, including:
- Conducting targeted outreach presentations for Veterans
- Keeping a relationship with a designated VA contact at the nearest VA medical or community clinic
- Integrating the Military History Checklist into our admission processes
- Maintaining policies and procedures that underscore our commitment to serving veterans
